Yes, you can have as many premium servers as you want.
At the first day of the month we reset all points from all servers, however this does not mean that all servers will end up having zero points on the first day of the month. All servers still receive their points based on yesterdays playercount and premium servers will receive their monthly points on top of that so they can have a headstart.
Yes, you can cancel your subscription any time. You will not get billed again after cancellation.
To upgrade or downgrade a subscription plan you will have to cancel your existing subscription from within your PayPal account and wait for it to expire, then you will need to re-subscribe to your desired subscription plan.
To cancel your PayPal subscriptions, log in to your PayPal account and click the gear icon at the top right corner. Go to Payments-->Manage automatic payments. Click the subscription name and click Cancel in the Status section.
